Patient Financial Policy (Summary)

At Integrated Lab And Health, we believe in transparent communication about the cost of care. Please review our financial guidelines below:

• Insurance: We accept most major insurance plans and will bill your insurance as a courtesy. You are responsible for any copays, deductibles, coinsurance, or non-covered      services.
• Payment: Payment is due at the time of service. We accept cash, credit/debit cards.
• Balances: You are responsible for any unpaid balances. Accounts past due may be subject to late fees or collections.
• Cancellations: Please give at least 36 hours’ notice if you need to cancel. Missed appointments or late cancellations may result in a fee.
• Labs & Testing: Some lab or diagnostic services may be billed separately by outside providers.
• Financial Assistance: If you are experiencing financial hardship, please contact us to discuss payment options.

By receiving services at Integrated Lab And Health, you acknowledge financial responsibility for charges not covered by insurance.

Scheduling & Cancellation Policy

We value your time and are committed to providing quality care to every patient. To help us serve you better, please review our scheduling and cancellation policies:

1. Scheduling Appointments

• Appointments may be scheduled by phone, online, or in person.
• Please arrive 10–15 minutes early to allow time for check-in and paperwork.

If you are running late, please call us. Patients arriving more than 15 minutes late may need to reschedule.

2. Cancellations & Rescheduling

• We require at least 36 hours’ notice to cancel or reschedule an appointment.
• Cancellations with less than 36 hours’ notice may be subject to a late cancellation fee depending on the service booked.
• Missed appointments (“no-shows”)  may also result in a fee and could affect your ability to book future visits.

3. Respecting Everyone’s Time

Your appointment time is reserved specifically for you. Late cancellations and missed appointments prevent other patients from receiving care. We appreciate your cooperation in helping us provide the best access to care for all.

4. How to Cancel or Reschedule

Please call our office during business hours or use our online scheduling system (if available). Messages left after business hours will be considered received the next business day.
